An important paralog of this gene is SIGLEC6.ialic-acid-binding immunoglobulin-like lectin (Siglec) that plays a role in mediating cell-cell interactions and in maintaining immune cells in a resting state. Preferentially recognizes and binds alpha-2,3- and more avidly alpha-2,6-linked sialic acid-bearing glycans. Upon engagement of ligands such as C1q or syalylated glycoproteins, two immunoreceptor tyrosine-based inhibitory motifs (ITIMs) located in CD33 cytoplasmic tail are phosphorylated by Src-like kinases such as LCK (PubMed:28325905, PubMed:10887109). These phosphorylations provide docking sites for the recruitment and activation of protein-tyrosine phosphatases PTPN6/SHP-1 and PTPN11/SHP-2. In turn, these phosphatases regulate downstream pathways through dephosphorylation of signaling molecules. One of the repressive effect of CD33 on monocyte activation requires phosphoinositide 3-kinase/PI3K.Structurally, CD33 contains a V-set Ig-like domain (IgV) and a C2-set Ig-like domain (IgC) following a signal peptide at the amino terminal. Given its expression on acute myeloid leukemia (AML) blasts in 85-90% of all patients, CD33 is the immunotarget of gemtuzumab ozogamicin (GO; Mylotarg™), an anti-CD33 antibody carrying a toxic calicheamicin-γ1 derivative. Upon binding to CD33 IgV domain, GO is internalized and the subsequent intracellular hydrolytic release induces DNA single-/double-strand breaks, apoptosis, and cell death. rs12459419 (C>T; A14V) is a single nucleotide polymorphism (SNP) which results in exon 2 skipping to yield a shorter isoform termed CD33-D2 or CD33m. This polymorphism is associated with response to GO, with a significantly lower risk of relapse seen only in patients expressing full-length CD33 (CC; 51%), but not in patients expressing CD33-D2 (CT/TT; 39%/10%) in the COG-AAML0531 trial (PMID28644774 ).
